Under the strict security protocol Apple imposes on Mac App Store applications (the Sandbox), an app cannot freely access your system directories without your consent. By selecting those two folders on first launch, you create a secure access (a Folder Bookmark) that lets the app scan and clean exactly those caches.
None. Xcode Cleaner never touches your project files, source code (.swift, .m, .h) or configurations. The app targets only temporary data directories (Derived Data, local SwiftPM caches, simulators and DeviceSupport). Xcode automatically recreates any missing file on your next build.
By removing Derived Data and the Swift Package Manager cache, Xcode has to recompile every file in your project and download its external dependencies again on the next build. This is perfectly normal and temporary.
Xcode Cleaner lists the system simulators downloaded by Xcode. In the dedicated section of the list, tick the runtime you want and click the red button at the bottom, or right-click the runtime to delete it individually.
